ATR is no longer required to provide a copy of 1095 forms to employees.

Employees can request a copy by contacting the office at 419-991-1010 and ask for
Human Resources, or email hr@atrcontractors.com. ATR will furnish this within 30 days
of the request (or by January 31, whichever is later).
